Insurance and Fees

I am in-network with Regence BlueCross BlueShield, Moda and Pacific Source insurance companies.  If I am out of network with your insurance, I am able to provide you with an invoice that you may submit to your insurance to request reimbursement.  Payment for out of network care is due at the time of your visit.
