Insurance and Fees

I am in-network with most Regence BlueCross BlueShield, Moda, Pacific Source and Providence insurance plans.  I am not currently in network with any OHP/Medicaid plans.  If I am out of network with your commercial insurance plan, I am able to provide you with an invoice that you may submit to your insurance to request reimbursement.  Payment for out of network care is due at the time of your visit.
